Hi. (1) I would like to know if there are any documented cases whereby someone got syphilis by kissing or deep kissing? If there is, does it mean we should not kiss or deep kiss someone?
(2) I recently went to a local polyclinic to get tested for syphilis. After i got my results, it showed 'ORDERED VDRL, TPPA
VDRL NON-REACTIVE, TPPA NON-REACTIVE.'
I have read your webpage and some other sources and it says that VDRL is a screening test and TPPA or TPHA is done only when there is a positive screening test (VDRL). So why was TPPA done when the VDRL was non-reactive? Or is it a standard protocol to do both VDRL and TPPA together?
(3) Could it be because i have a 'mild or weak' positive so they decided to do TPPA also? The doctor at the polyclinic did not say anything about me having mild or weak positive he just said my test result is normal. The blood test result also did not show if i had mild or weak positive it just showed NON-REACTIVE for both VDRL and TPPA. So what is the case now?
(4) May i know what is your protocol for testing syphilis? If lets say someone has VDRL NON-REACTIVE, do you stop there and conclude or u go on and do confirmatory tests like TPPA or TPHA? Do different medical institutions have different set of testing for syphilis? I'm very worried now and i dont know if i'm just scaring myself & i appreciate it if you could reply. Thank you! |

It is unlikely to get STI and HIV through deep wet kissing or French kissing, unless there is bleeding gums or ulcers in both parties and even this is considered a low risk. There is no documented case of someone infected through this mode.
There are several testing procedures to determine a Syphilis infection. Since you had taken both the basic and confirmatory screening, it shows that you are not having the infection provided that you had taken this after 3 months from the exposure. You do not need further testing. |

i m syphilis positive. i have rash at my back of my body and palm and sole. i have taken 1st injection of penicillin and i am going back for 2nd injection next week. the problem is, my rash still continue to develope on my hand after the 1st injection.. what should i do? i m scare... my shoulder back is numbness and paralysis... and i dun know which stage is my syphilis at now.. becaue the doctor in SGH did not advise me.. and i always feel very tired... and restless... pls urgently advise me.... |

Hi,
I'm now pregnant. I was tested positive for syphilis and had received the appropriate treatment. I believe my husband has also contracted the disease but he has not gone for the test yet. Recently, we had a light petting. Does sucking of breasts consider oral sex? Will it pass the disease back to me? Also, do i need to go for the HIV test too as i am not too sure if my gynae has tested HIV for me at the time when he tested for syphilis? |

You do not risk a re-infection, however, your husband needs a test to exclude the infection. Meanwhile, abstain from sex till your husband has taken the screening. You will need regular follow up for the condition and you may wish to check with your doctor about your HIV status. |

It does not mean that contact with an infected partner will be transmitted with STI including Syphilis. There is a potential risk but the actual risk is still subjective at the point of contact. For example, use of protection, length of contact, activity of the micro-organism etc. |

Once appropriate treatment like Benzathine Penicillin, Procaine Penicillin had been given, the person would normally be cured of Syphilis. Other drugs are normally used for patients who are allergic to Penicillin based treatment. However, subsequent blood test will still remain positive especially the TPHA or TPPA test. This test is one of the confirmatory test for Syphilis. The basic screening test like VDRL or RPR may show positive, weak positive or negative. So long as the subsequent VDRL or RPR titre drops and/ or does not reflect higher indication prior treatment, the person is deemed to be cured and there is no active Syphilis. These tests are reflections of the body's responses and the indication will be shown forever. |

I had protected sex but not protected oral sex. 5 weeks later went for AFA HIV blood test and it came back negative. Another 2 weeks later I visited DSC for syphillis and HIV blood test.Both came back negative.Today 13 weeks after the incident I went for AFA blood test again,result was negative. My question is, do I have to go for the syphillis and HIV test again? Pls note that I only did the syphillis test once. I'm tested negative will I still be infected with HIV? A blood test for Syphilis and HIV can be taken 3 months after the exposure for a conclusive result. Hence, you may wish to repeat both Syphilis and HIV. |

In marth this year i discovered that i had syphilis cos my gf tested positive ,and my RPR was 1:64 ,TPPA was positive.I suspect i might have acquired the disease last year in December cos i had some encounter with someone else.I was given 4 penicillin ejections with some oral medicine during march this year.The problem is I have to go to work in NewZealand at the beginning of July this year and they will need to test my blood for Syphilis ,what should i do to convince them that i have already taken the medicine and am responding well.I don't have any symptoms and i feel nothing at all. |

You can ask for a documentation of your status from the doctor that had tested and given you the treatment. The memo or letter can state the condition before treatment, the blood test result, the type of treatment given, and the status after treatment if any. |

Syphilis can be transmitted through these ways:
- unprotected sexual intercourse with an infected person (vaginal, anal, oral)
- mother-to-child (transplacental infection) during pregnancy
- blood transfusion with contaminated blood products
- contaminated needle stick injury and sharing intravenous needles with an infected person
Your report did not indicate that you had previously contracted Syphilis. This may represent biological false positive for various reasons or most recent Syphilis infection. We advise you to be followed up by your doctor or STI specialist to determine the factors for this status.
Your boyfriend's blood test should include a TPPA or TPHA test if you are positive for Syphilis. |

I am tested positive for TTHA-VDRL on routine screening during my pre-employment medical examination. I started treated for Chronic Latent Syphilis March 9, 16 & 23, 2007, & had Benzathine Penicillin 2.4 Million units by deep intramuscular injection.
On July 17, 2007 had another TTHA-VDRL test & result – positive & titire 1:8, then again another test on January 14, 2008, result – positive & titire 1:2.
My Infectious Disease specialist doctor advice me to have another test after few months again, to determine if I’m then negative or weak positive. The reason why I will go to Singapore and have my routine test in your clinic
My concern is that, if my result in your test is weak positive or Negative, can your clinic issue us certificate that I am now cured & fit to work? I will use it as supporting documents once I had medical test in our country.

I discovered a red peeling spot on three days ago. I suspected it to be a syphilis sore. The last sexual exposure was 149 days before this sore appeared.I had recurring fungal infection at the penis area these few months and i had been scheduled for a circumcision soon. I immediately went to the DSC yesterday to see the doc. The nurse and doc told me that it is highly unlikely to be a syphilis sore without doing any test. I also did a HIV rapid test last week which was negative. I want to know whether is it possible for the sore to appear on the penis 149 days after sexual exposure. And how can the doc confirm it is not syphilis without doing a blood test? Is the HIV rapid test a confirmatory result 140 days after the last sexual encounter? I heard that 6 months is the window period. Hope you can help me. Thanks |

As you had visited the DSC Clinic, you can be assured that your case would not be linked to Syphilis. Generally, the sore would appear in a few days to weeks after the exposure. For your case, it was already 149 days. Moreover, our nurse and doctor had seen that the sore would not be typical of a Syphilis sore.
If you need further assurance, you can return to do a blood test for Syphilis and the result can be conclusive if there is no new exposure.
We would like to state that the window period for HIV is between 1 to 3 months after an exposure. A person can go for a HIV ELISA or Rapid test earliest 1 month after an exposure and retest at the 3rd month for a conclusive result. Most people would be negative when the HIV test is repeated at the 3rd month. It is rare that people shows positive at the 6th month.
A test at the 3rd month would be good to conclude. |
